POSITION: Community Follow-Up Worker
SUMMARY OF POSITION:
The Community Follow-Up Worker provides a wide range of direct and indirect support services to at-risk or HIV positive clients with the goal of connecting clients with community services, medical services and entitlements.
ESSENTIAL FUNCTIONS:
- Assists case management team with case management skills and data entry work
- Conducts preliminary interviews/ screening for program
- Escort client/ families to appointments when necessary, accompany staff when necessary
- Make extensive home and field visits to contact agencies, do outreach, case finding, and locating clients ensure program participation and accessing of services. Identify resources and promote program
- Assist in office administrative work. Monitor completion of forms and records
- Assists clients with home making, nutrition, budgeting and parenting. Identify and report client needs/stressors
- Act as liaison between the client, family and the case manager to ensure client trust and confidence
- Assists in actual child care where there is an emergency, otherwise arrange for services
- Performs other duties as assigned or requested during emergencies

EDUCATION/EXPERIENCE:
Ability to read, write and understand English and carry out directions. HS Diploma or GED preferred. One year experience working with HIV/ AIDS a plus and substance abuse population. Must have good written and verbal communication skills. Bilingual English/ Spanish preferred.
