POSITION: Licensed Master Social Worker
LOCATION:Brooklyn and Queens Location
SUMMARY OF POSITION:
Coordinates and oversees all aspects of psychosocial programming. Assists in planning, developing, organizing and evaluating psychosocial, as well as, concrete needs of program participants.
ESSENTIAL FUNCTIONS:
- Assists in planning, developing and evaluating the psychosocial needs and concrete needs of program clients
- Ensures that admission and discharge procedures are consistent with individual needs
- Ensures completion of psychosocial assessments of clients within the mandated time frame
- Participates in the interdisciplinary team care planning process
- Participates in the interdisciplinary team care planning process
- Carries a caseload
- Serves as a resources for all other program staff; develops and maintains resource file
- Leads or co-leads psychosocial groups
- Performs interagency liaison work with other service agencies, as needed
- Maintains client case notes and statistical records; ensures staff documentation of progress notes
- Participates in Quality Assurance program to evaluate, monitor, identify problems and institute revisions to maintain quality of client care and provide services in accordance with current standards of professional practice
- Attends and participates in seminars and continuing education programs to expand knowledge of current trends in the field of specialty and in issues concerns of the HIV population
- Attends all mandatory in-service programs
- Maintains all confidentiality of all pertinent
- Works on a rotated Holiday Schedule

EDUCATION/EXPERIENCE:
MSW from an accredited school of social work; licensed by the State of New York; fluent in both oral and written English; experience in public health within the HIV/ AIDS and substance abuse population.
