Adult Day Health Care | Residential Health Care and Drug Treatment | Primary Care | COBRA Case Management
COBRA provides intensive case management services to HIV-infected and high risk persons, who are identified as having difficulty accessing medical care and/or other services, and who require frequent personal contact and/or home visitation to facilitate and ensure their compliance with medical care and other needed services.
How COBRA Case Management Works
Each patient is assigned to COBRA Case Management Team of three experienced professionals working collaboratively with the patient. The Case Manager leads the team (which includes a Case Management Technician and a Community Follow-up Worker) working closely with each client to ensure basic rights, such as temporary shelter, permanent housing, entitlements, food and health care are obtained.
Services
- Assessment of Client’s Biopsychosocial Status
- Continuous evaluation of client progress toward stated goals and service plan development
- Implementation of service plan goals in collaboration with client to achieve stability
- Continuous Case Monitoring and Follow-up
- Crisis Intervention if Needed
- Discharge Planning to Appropriate Level of Care
Criteria to Participate in PSI’s COBRA CASE management program
- HIV+/AIDS diagnosis or determined to be at High Risk for contracting HIV. Family members and other collaterals of COBRA clients may also be approved to receive case management services, as deemed necessary.
