HELP/PSI operates the largest Comprehensive Medicaid Case Management Program in the State of New York. Commonly known as “COBRA,” the program provides intensive case management services to HIV-infected individuals and people at high risk of contracting HIV in the Bronx and Queens. COBRA assists these individuals, identified as having difficulty accessing medical care and/or other services, and who require frequent personal contact or home visitation to facilitate and ensure their compliance with medical care and other needed services.
HOW Health Home (COBRA) WORKS
Each patient is assigned to Health Home (COBRA) Team of three experienced professionals working collaboratively with the patient to obtain primary medical care and achieve other identified goals. The Case Management team works diligently with each client to ensure basic rights, such as temporary shelter, permanent housing, entitlements, food and health care are obtained.
Health home (COBRA) Services IN THE BRONX AND QUEENS
- Assessment of client’s biopsychosocial status
- Continuous evaluation of client progress toward stated goals and service plan development
- Implementation of service plan goals in collaboration with client to achieve stability
- Continuous case monitoring and follow-up
- Crisis Intervention if needed
- Discharge planning to appropriate level of care
CRITERIA TO PARTICIPATE IN HELP/PSI’S Health Home (COBRA) PROGRAM
- HIV/AIDS diagnosis or determined to be at high risk for contracting HIV. Family members and other collaterals of COBRA clients may also be approved to receive case management services, as deemed necessary.
